With 24-hour residential care, West Tennessee Family Solutions provides inclusive support and services for individuals with intellectual and developmental disabilities and their families. Our exceptional staff and caring team works with families to ensure each individual has a productive day, gains independence and makes personal choices in their day’s schedule.

Supported Living

Individuals benefit from supported-living services because:

  • Community homes with two or three residents provide family and community interaction.

  • Staff provides one-to-one programming and instruction for each individual’s needs and preferences, 24/7.

  • Supported living fosters individuals’ full participation in the community as they work toward long-term personal goals.

Day Services

Spending time with friends and being part of the larger world engages individuals while reinforcing life skills. Our day adventures include shopping, community events, eating out and going to the movies.

Individuals may want to create the day as they go. That’s OK too. Others may need picture books or other means of communication to help in planning their day. That’s why we’re here. West Tennessee Family Solutions’ staff will present options and help choose activities to ensure a constructive experience.

Nursing

No matter the day or the hour, our nurses stand ready to provide care. As individuals’ health advocates, nurses monitor prescribed medications and ensure that medication refills are current and picked up on time from the pharmacy. They also perform head-to-toe health checks monthly, more often if needed, and will schedule doctors’ appointments, attend appointments as needed, and stay in touch with individuals’ doctors and healthcare providers, if necessary.

West Tennessee Family Solutions employs LPNs who are supervised by an RN.

Supported Employment & Job Coaching

We believe all people should have a chance to work and be compensated for their work. More than 15% of our individuals have competitive jobs in the community.

On the job, most individuals have a coach to train, teach and encourage them to complete duties independently so they feel successful and proud of their work. The coach also will advocate for the individual, if necessary.

Respite

West Tennessee Family Solutions provides short-term support for those who qualify—temporary relief for families who need support and individuals who may need care for other purposes.
